We have per diem, part time, and full time positions available
throughout King and Snohomish counties.
This position is responsible for assessing, planning, implementing,
evaluating and providing skilled care to patients requiring physical
therapy services in the home in conjunction with the patient, family and
the interdisciplinary team, as ordered by the physician. The PT will be
responsible for clinical outcomes as well as industry benchmarks.
Home Health care is performed independently, in the patient's own home
environment. To be successful, clinicians must be organized, flexible
and creative, able to adapt to a wide variety of home environments and
fluctuating schedules. Physical therapists provide, coordinate and
oversee skilled intermittent care to patients and include instruction
and education of patients and/or caregivers to promote maximal
functional status.
JOB REQUIREMENTS:
BA/BS; Master's; or certificate in Physical Therapy
required. Minimum two years recent experience in acute care and/or
rehabilitative center preferred. WA Physical Therapist license and
Healthcare Provider CPR at date of hire.
Must also have valid WA state driver's license and auto insurance for
Home Health positions.
